Which food services passed or failed their health inspection May 26 – June 1

SYRACUSE, N.Y. (WSYR-TV) — The Onondaga County Health Department released its inspection reports for restaurants checked from May 26 through June 1.

Two food services failed:

• Don Juan Cafe – 102 Grand Avenue, Syracuse
• Thai Flavor – 2863 Erie Boulevard East, Syracuse

Don Juan Cafe

Don Juan Cafe had 9 violations, with one being a critical violation.

Improper cooling and refrigerated storage potentially hazardous foods (critical violation) – The inspector found several protein products stored at 48-52 degrees Fahrenheit instead of 45 degrees Fahrenheit as required. The operator said they were unaware of temperature requirements.

A box of chicken was found at 40-42 degrees Fahrenheit and was moved to the cook line double-door refrigerator.

The following products were voluntarily discarded:

• Approximately 20 lbs. of pork shoulder
• 10 lbs. of ground beef

Food not protected in general (1 violation) – The inspector found bulk food containers near the defective refrigeration stored directly on the floor.

Poor sanitary design, construction, installation of equipment and utensils (1 violation) – The inspector found a cutting board with heavy staining and deep scars.

Poor sanitary design, construction, installation of equipment and utensils (1 violation) – The inspector found the glass door refrigeration in the cook line room with damaged shelves.

Improper cleaning, washing and sanitizing of equipment and utensils (1 violation) – The inspector found interiors of defective refrigeration with liquid blood residues, gasket material to all refrigeration with food residues, interior surfaces of cook line flip top with loose food debris, cook line equipment with dried food residues and loose debris.

Improper sanitary facilities and controls (1 violation) – The inspector found a leak at the three-bay sink was being captured with a steel pan.

Improper garbage and rubbish disposal (1 violation) – The inspector found dumpsters uncovered with birds, squirrels and heavy house flies present.

Improper construction and maintenance of physical facilities (1 violation) – The inspector found floors, under and behind stationary cookline equipment with dried residues at wall edges. Floors and walls, drain pits and surfaces in sanitation areas with food residues.

Thai Flavor

Thai Flavor had 14 violations, with three being critical.

Food not protected from contamination by other sources (Critical violation, found two times) – The inspector found numerous secondary spray bottles of various toxic chemicals in the kitchen and dry storage was unlabeled. This was corrected. Sharpie was given to the operator and all bottles were labeled for identification of contents.

At the cook line, a bottle of quaternary sanitizer tablets was found. Sanitation tablets were moved to the three-bay sink.

Food not protected from contamination by other sources (Critical violation, found one time) – The inspector found in the walk-in cooler, an open bulk container of bean sprouts in water at risk of contamination from extremely corroded wire shelving. This was corrected.

Poor sanitary design, construction, installation of equipment and utensils (1 violation) – The inspector found cutting boards aged with scars and stains.

Poor sanitary design, construction, installation of equipment and utensils (2 violations) – The inspector found in walk-in refrigeration, wire shelving is corroded and risks adulteration of foods. Cardboard boxes are used at the cook line to store squeeze bottles and spices, not an easily cleanable surface.

Improper cleaning, washing and sanitizing of equipment and utensils (1 violation) – The inspector found that the automatic dishwasher was not dispensing sanitizer additive. Several attempts were made to prime the system. The operator advised to arrange for a professional service. Chlorine content must achieve 50 ppm chlorine at the rinse cycle. Quaternary sanitizer was found. Serviceware will be sanitized in the three-bay sink until automatic dishwasher repairs are complete.

Improper cleaning, washing and sanitizing of equipment and utensils (5 violations) – The inspector found the cook line refrigeration was unclean. All shelving in refrigeration and dry storage is unclean. Lower shelving of the preparation table at the cook line with heavy food residues. The exterior of all bulk storage containers is scuffed and unclean. The cookline equipment had food debris and dried residues. All plumbing and gas service runs are soiled and have dried food residues.

Secondary squeeze bottles of food products were soiled and unable to be labeled.

Improper sanitary facilities and controls (1 violation) – The inspector found the drain from the dishwasher is being supported by a tin can (photo) Slope also appears to be backward for proper wastewater flow.

Improper construction and maintenance of physical facilities (3 violations) – The inspector found all floors in the kitchen area with soil and food debris. Floors in walk-in refrigeration and freezer with food residues. Floors throughout the back house are in poor repair and soiled. Walls in sanitation area with stains and dried food residues.

Improper construction and maintenance of physical facilities (1 violation) – The food inspector found in the kitchen production space, numerous tools and materials not associated with food service on shelves and spaces.

Many locations passed their inspections between May 26 through June 6.
